The medical criteria

Universal candidacy.

These are the standard medical criteria for bariatric surgery — same as the U.S. consensus, same as our criteria. If you check at least one box, you almost certainly qualify.

35+

BMI 35+ alone

A BMI of 35 or higher with no other conditions is enough on its own to qualify for bariatric surgery under modern guidelines.

30+

BMI 30+ with comorbidity

A BMI of 30+ combined with type-2 diabetes, severe GERD, sleep apnea, hypertension, or significant joint pain qualifies under current ASMBS guidelines.

18–70

Age range

Standard age range is 18–70, though we evaluate older adults case-by-case based on overall health and life expectancy.

No prior diet requirement

Unlike U.S. insurance, we don't require a 6-month supervised diet history. You've probably already tried — that's why you're here.

Cleared for general anesthesia

You'll need basic pre-op clearance. We handle EKG, blood work, and anesthesia evaluation as part of your package.

Honest disclosure

When we say no.

There are real medical reasons we'll decline a candidate. We'd rather lose your business than put you at unacceptable risk. These are the absolute disqualifiers.

Active cancer (with rare exceptions)
Active malignancy — particularly during chemo or radiation — is generally a hard no. We typically wait until 2 years post-remission before evaluating, depending on cancer type.
Severe untreated heart disease
Recent MI, severe CHF, unstable angina, uncontrolled arrhythmia — these dramatically raise anesthesia risk. We require cardiology clearance and stable management first.
Pregnancy or planning pregnancy < 18 mo
We don't operate on pregnant patients. We also recommend waiting at least 12–18 months after surgery before becoming pregnant due to nutritional demands.
Active untreated substance abuse
Active alcoholism or untreated substance use disorder. We require demonstrated sobriety (typically 6–12 months) before evaluating.
Severe uncontrolled mental health crisis
Active suicidality, severe untreated bipolar/psychotic conditions, or untreated severe eating disorders. We require psychiatric stability before surgery — your mental health team should be on board.
Inability to commit to follow-up
Bariatric surgery requires lifelong nutrition, vitamins, and follow-up. If a patient can't or won't commit to that, the surgery becomes dangerous rather than helpful.
If we say no

There's almost always a path forward.

A "no" today often becomes a "yes" in 6–12 months. Here's what we typically recommend depending on the reason — and we'll tell you exactly which path applies to your case.

  • Stabilize the medical condition (cardiac, oncologic, psychiatric) and re-evaluate
  • Document substance recovery (typically 6–12 months sobriety)
  • Wait through a planned pregnancy and post-partum window
  • Address an eating disorder with a specialized treatment team
  • Explore non-surgical options (medications, behavioral programs) as a bridge
